What Every New Client Should Know About Their First Mission Visit

Mission Northeast provides programs and services for the following zip codes:   77302, 77303, 77306, 77327, 77328, 77336, 77339, 77345, 77357, 77365, and 77372
A person in need of services must personally come in to our office located at 22098 Loop 494, New Caney during the hours of 9 and 11, Tuesday, Wednesday, Thursday.  We do not accept phone, email or third party applications.
Here’s what you will need on your first visit:
Proof of residence for EVERYONE who lives at your address.  As an example, you would need at least one of the following:
Valid drivers license with current address
Current utility bill
Current mail
Lease agreement including all resident names
AND
 
Proof of income.  Provide one of the following:
Current pay stub
Copy of your latest tax return
Current Disability Income Award Letter
AND
 
For each child at your address, you will need one of the following:
Medicaid Letter
CHIPS card
Shot record
Letter from Doctor
Enrollment letter from school if they are school age
Crib Card for babies
